Sarah Hamm
Geriatric Nurse Practitioner
About Sarah
Sarah has been with our team since early 2021 and was the first provider hired to join our practice! Sarah has specialized in geriatric care since receiving her Master of Science in Nursing in 2016.
Like our broader practice, she follows a palliative style of care, focusing on improving our clients' comfort and quality of life. She is passionate about promoting the health and independence of seniors as they age. Sarah has extensive experience managing complex chronic conditions, including dementia, diabetes, depression, heart failure, and many other issues our clients often face.
Before working with Pathway Geriatrics, Sarah was part of a private practice providing care in skilled nursing facilities. She has also worked in short-term rehab and long-term care settings.
Sarah lives in North Bend with her husband, two children, two dogs, a cat, and a small flock of chickens. She loves spending time outdoors and is an avid runner, formerly competing in track and cross country for Boston College.
