We Bring Healthcare to You
Pathway Geriatrics provides in-home primary care services to homebound older adults on the Eastside of Seattle
As your Primary Care Provider, we help manage your healthcare and comfort as you age. Our providers meet you in the comfort of your home, and offer the same services as a normal provider.
We currently limit our services to Adult Family Homes or Assisted Living Facilities.
Services that Extend Beyond Each Visit
In addition to our base In-home Primary Care offering, our clients and their caregivers have access to a number of additional services to extend coverage between visits.
Care Coordination
We coordinate with caregivers, doctors, powers of attorney, and other family members to provide the kind of healthcare that helps our clients get the most out of life.
Regular Checkups
We try to be in each home or facility area at least once every five to six weeks for medically necessary visits.
24 x 7 Support
We provide after hours phone support for urgent care inquiries to qualified caregivers.
Health Monitoring
Our providers coordinate with qualified caregivers to ensure that our clients get continued care between their scheduled checkups.
Dementia Care
Our providers have extensive experience caring for clients with all types of dementia.
Wellness Education
Change is scary. We help educate our clients and their loved ones about what to expect as they age, and how to best manage the effects.

Our services are billed through your insurance, just like any other doctor’s visit. There are no hidden costs or fees for our services.
Fewer ER Visits
ER visits are expensive and stressful. Our regular checkups and urgent care support for caregivers are aimed at reducing unecessary trips to the ER.
Improved Mental Health
Mental health is just as important as physical health. Our providers are trained to look beyond a patients physical needs, taking into account both their health and happiness.
Environmental Observation
Many of our clients are unable to communicate their needs. By seeing a patient in their home, we can greatly improve our care plans through observing actions and behaviors that are unique to that environment.
How it Works
1We become your Primary Care Provider
Your primary care provider is like the quarterback of your healthcare team. Our job to to see the big picture of your health, and coordinate with the specialists, caregivers, and other parties involved in your care. Our goal is to take a holistic look at your well being, work with you to pull together a gameplan, and then make sure everyone involved is doing their job well.
2We schedule regular visits to your home
Our providers are typically in your area every five to six weeks. We coordinate with you and your caregivers to determine whether you need to be seen or not. Each checkup is just like a checkup to your current primary care provider, except that we can infer a lot more about your needs by meeting you in your home.
3We coordinate with caregivers between visits
Unfortunately our health doesn’t abide by a schedule. So we continue to collaborate with your caregiver between visits. If something comes up that is out of their field of expertise, they can give us a call to help coordinate the best plan of action. We try to make ourselves available to them 24 x 7, within reason.
4We keep your loved ones in the loop
With your permission, we can keep one or more parties up-to-date with your health status. We offer an always-available patient portal to the client or power-of-attorney, and are happy to provide additional communication upon request.

When we are young, it makes sense to fight for a longer life, even when it leads to discomfort. As we near the end of our life, our comfort takes precedence. We work with our clients to strike the right balance between living longer and living better.
Often the Best Medicine is the One Removed
Medication is an important part of proper health management, but too much can be detrimental to our health. We prescribe to a philosophy of keeping medications to the minimum required for your health and comfort.

Our service offerings are currently limited to licensed Adult Family Homes and Assisted Living Facilities.
Our service area at present covers the greater eastside of Seattle, including Bellevue, Redmond, Kirkland, Woodinville, Bothell, Lynnwood, Edmonds, Mercer Island, Issaquah, Sammamish and Renton.
